WISCPHR Award of Excellence

Each year at the Annual Conference, WISCPHR awards the "Award of Excellence" to members who are outstanding professionals and who have served the Society as mentors and leaders. Applications are Now Open. Deadline to Nominate: March 15, 2019.

WISCPHR Innovation Award

Each year at the Annual Conference, WISCPHR awards the "Innovation Award" to a program that has enhanced their delivery of care for a patient population, in a unique and creative way, beyond the traditional model of Cardiac and Pulmonary Rehabilitation. The following programs have received this award:

2017 - Flambeau Hospital's Pulmonary Rehabilitation Program- Harmonica Training
2016 - St Elizabeth's Hospital-"Breathe at Ease" COPD Care-Management Program
2015 - Aspirus Wausau Hospital-ADAPT (Aspirus Diabetes, Assessment, Prevention, Transition Program)
2014 - Upland Hills Health-Cancer Empowerment Program and Baldwin Area Medical Center-Heart Failure Program

Presidential Citation & Recognition Award

Each year at the Annual Conference, the outgoing WISCPHR President has the opportunity to formally recognize an individual the "Presidential Citation & Recognition Award" who has made significant contributions to the president's term and the association as a whole. Distinguished Service Award

Each year at the Annual Conference, the WISCPHR Executive Committee has the opportunity on any given year to award the "Distinguished Service Award" to an individual whose leadership, ideas, and committee work have significantly benefited WISCPHR as an organization. Honor of Clinical Distinction

The Honor of Clinical Distinction is given in recognition of an outstanding clinician in the field of cardiac and/or pulmonary rehabilitation. This award is intended to identify an individual who is highly effective in improving the outcomes of their patients by applying superlative clinical vigilance and motivational techniques. The candidate should also be considered a resource for program guideline clarification and other regulatory bodies. Applications are Now Open. Deadline to Nominate: March 15, 2019
